Kouper emerged from stealth for $10 million to fix a patient transition, one of the most overlooked mistakes in healthcare.

Kouper, a new health technology startup focused on making hospital discharges safer and smoother, has officially launched it with $10 million in fresh funding. The round was led by General Catalyst with participation from 25 Madison and CVS Health Ventures. The news lands amid growing interest in AI-driven health technologies.

For decades, handoffs between hospital care and what comes next have been one of the most vulnerable moments in a patient’s journey. It’s also where things often fall apart. Inadequately coordinated discharges lead to confusion, missed follow-up, consequences, avoiding outcomes, and cost over $53 billion per year in avoidable readmissions. Kouper is trying to stop the spiral before it begins.

Kouper is launched for $10 million and fixes healthcare’s most broken moments: hospital discharge

Founded by CEO Salman Ali (co-founder of GetSnooze) and CTO Dr. Ablimit Keskin (formerly of Komodo Health), Kouper builds software that connects directly to hospitals and health systems. After being discharged from a hospital, ER, or emergency care, patients will continue to engage with AI in real time to schedul the scheduling, surface red flags. And do all this without adding any more work to the clinical team.

With fresh funding, Kouper is expanding his team and deepening his work with early health system partners.

“Traveling through the American healthcare system is full of challenges and can be particularly challenging during the transition from hospital to residential environments,” said Salman Ali, co-founder and CEO of Kouper. “The combination of the repetition between our team’s living experience and the health of our incubation partner 25m has placed an absolute focus on creating a personalized transition of Kouper’s foundation and care experiences for all patients leaving the hospital. Our team and customer base are growing rapidly, and the verification received from our healthcare system customers is incredibly encouraging.”

What Kouper is really building is plumbing for better handoffs in healthcare. Its platform provides a more complete, real-time view of each patient after hospital and care teams have been discharged. Instead of fragmented spreadsheets or disconnected systems, Kouper can bring everything to one place and make decisions quickly based on the exact context.

Also, there is no need for large teams or large implementations. The hospital can start Kouper within a month. According to the company, some partners have already jumped over 50% patient involvement, and follow-up appointment rates have increased by more than 60%.
