Deceased: A 39-year-old woman in Belgrade, Serbia

Discovery: The woman’s ex-husband finds the woman’s body in her apartment and notifies the police. She was lying on the living room sofa to her left, her body covered in a blanket.

What happened next: Police removed the blanket to check on the body. The woman was naked from her waist. There were no visible signs of violence or injury. Police then discovered the woman’s object protruding from the bottom.

The wooden handle was inserted into the anus to a depth of about 4 inches (10 cm) into the anus, penetrating the anus and rectum as “dildo-like objects,” doctors wrote in their report of the incident. According to her ex-husband, the woman was hypertensive, also known as arterial hypertension, and was taking medication.

Autopsy Findings: Examination of the female skull revealed bleeding in the space between the brain and the surrounding tissue layer. Blood was pooling up to 0.2 inches (6 mm) deep around the brain and base of the brainstem.

When medical examiners cleared the blood clot, they discovered a ruptured sacred aneurysm, a sphere filled with bulging blood from blood vessels in the brain. It is also known as a “berry aneurysm” because it resembles a round berries hanging from the stem, and is the most common type of aneurysm.

The aneurysm was measured at a diameter of 0.4 inches (11 mm) and popped out onto the top of the dome. When an aneurysm ruptures, they kill nearby brain cells and release blood that can increase pressure within the skull. As pressure accumulates, the flow of oxygen and blood to the brain can be destroyed, which can lead to unconsciousness and death. Aneurysms are more common in women than in men, especially women with lower estrogen levels, especially postmenopausal women. People with high blood pressure are also more likely to develop an aneurysm that ruptures in the brain and causes bleeding.

Physical exercise increases blood pressure, and orgasms further increase that. In the case of women, her partial nude, body posture, and the presence of a whisker suggested that she was using kitchen tools for anal self-stimulation, the author of the report wrote. Her sexual arousal and subsequent orgasms could have caused blood pressure sufficient to rupture the aneurysm, causing fatal bleeding.

What makes the case unique: Sexual activity is not a common cause of sudden death. In a previous study, which evaluated a total of nearly 130,000 forensic autopsies, 195 deaths (0.15%) were gender-related, with only a small portion of the deceased being female. According to the report, when someone dies during or during sexual intercourse, it is usually an older man with a much younger female partner, and the cause of death is usually a sudden heart failure.

In women, sudden deaths in the sex are often caused by subarachnoid hemorrhage from a burst saccular aneurysm. However, this is the first case that presented such a scenario for the death of a woman during anal self-stimulation, the author wrote.
